Billing Checklist

Before your first appointment with William Colantoni Jr MD please review our billing checklist. 


We are happy to assist you in understanding your bill and navigating your out of pocket cost. If you are in need of assistance please contact our office at (563) 386-3333


Medicare coverage is accepted for people who qualify.

Dr. Colantoni Jr. does not accept Medicaid plans for any state.

You must have participating insurance coverage to schedule an office visit.


Ins Coverage: varies widely among ins carriers and within the plan the may offer. You are overall financially responsible for all charges incurred and this is subject to change based on type of service, and YOUR health insurance plan. 


A referral is not required for an appointment only if required by your insurance provider other than as required by your insurance provider. Before your appointment, contact your insurance company to determine your plan's requirements for specialty care. At check-in, co-payment is required at the time of visit.

Before Your Appointment


  1. Reach out to your insurance company for a benefits and your plan’s requirements explanation.
  2. Confirm that Dr. William Colantoni Jr. is a contracted provider of services for your plan.
  3. Confirm if your insurance company requires a referral. Discuss this with your primary care physician prior to scheduling your appointment.
  4. Know the limitations of the referral and obtain an extension or expansion of the scope of the referral if needed.
  5. If you are a new patient or have experienced a change in your insurance coverage since your last visit, please contact our office prior to your appointment.

At Check-In

  1. Present your driver’s license or another form of identification.
  2. Present your health insurance card.
  3. If you are a current patient, let us know if any personal or insurance information changed since your last visit.
  4. Be prepared to make a co-payment or deductible as needed.

After Your Visit

  1. Respond promptly to any requests from your insurance provider to ensure timely resolution of your charges.
  2. If you have an out-of-pocket balance due on a bill, please make timely payment.
  3. Call us if you have any questions or concerns about a bill or to make an billing arrangements.
  4. If deductible has not been met, this may need to be fulfilled at the time of visit as well.
  5. After your visit, call if you have any questions or concerns about a bill or to make payment arrangements.
